दोनों को इक अदा में रज़ा-मंद कर गई

Meaning
Your glance has pierced through
my heart to my liver.
It has made both of them agreeable
by just a graceful gesture (ada)

Explanation/Comment:

Ghalib describes what happened to him due to just a special look that someone gave him.

That special glance pierced through his heart and entered his liver. Meaning it made him fall in love instantly and also gave him the courage to move forward in love. (Heart is the seat of emotions, as liver is the seat of courage. And you need both in love.)

That special gesture and bewitching countenance (ada) made both his love (heart) and courage (liver) agreeable to everything to follow.

By just a look.

Fortunate are those who experience such a special glance (and ada) and fall head over heels in love.

More fortunate are those who can muster the courage to lead what follows to its logical conclusion.

Regardless of consequences!
